Episode 1 of Grey’s Anatomy Season 20 begins with Owen giving everyone a rundown on Teddy’s condition. He says that Teddy has now been resuscitated after collapsing in the OR. The echo done revealed bacterial endocarditis, which Owen believes is caused by a dental infection.

Owen continues that the infection caused severe aortic valve sufficiency, so Beckman and Schmitt had to do an emergency valve replacement. However, they still need to follow up on blood and tissue culture, and cardiology.

Amelia suggests Owen take a break, and spend some time with the kids. Winston also suggests the same and orders Schmitt to do post-op CT scans. Meanwhile, Nick addresses the interns and asks why they weren’t at the place they were supposed to be. In case you missed the events of season 19, the interns are under scrutiny after they made a hasty decision to open up a patient (Sam), leading to his death. Simone and Lucas try to help a crash victim by an ambulance driver, but the situation turns intense when they are trapped in the very same ambulance because a car crashes into the ambulance. They try to open the door, but it is in vain.

Meredith and Miranda arrive at the scene and ask Simone and Lucas to open the patient in the ambulance, or he’ll die. Link and Blue also join Miranda and Meredith outside. Miranda orders the interns to open the patient, but Simone is hesitant because of what happened to Sam. Lucas makes an incision, and Simone eventually takes over and manages to stabilize the patients.

Meanwhile, Jules goes to see Maxine and subtly reveals that she might be in love with Blue. However, Jules is also fearful of losing her job. She tries to convince Webber that she wasn’t involved in the tragedy, but he doesn’t listen, and says Jules’ name is on the patient’s medical chart, and she is absolutely involved.

Winston asks Schmitt to call upto IR and prepare for an endovascular embolectomy after noticing that Teddy has no blood circulation in her leg. The test reveals that Teddy has developed a clot, and when Winston asks Owen for his consent, he gives him the thumbs up.

Meanwhile, Meredith and Miranda also decide to scrub up to save the ambulance patient. We also see Nick, who is visibly upset because Simone and Lucas went against him and left the room. Simone asks permission to leave the OR, but Meredith orders her to stay, observe and learn. The operation goes well and the patient is saved. Winston also manages to take care of the clot and informs Owen that Teddy is out of danger.

Teddy eventually wakes up with her husband and the love of her life beside her. Nick again meets the interns and informs them that they’re not fired, but he is no longer going to work in the hospital as he is planning to move to Boston. Nick also tells everyone that despite what he said this morning, he still has total faith in them. As the episode nears its end, Nick leaves his interns in the capable hands of Dr. Miranda.
